How Do I Renew My License?

Application for Renewal License

All Designer Licenses will expire annually on December 31st.

To be eligible for license renewal, a person must:

  • Submit a completed and signed RENEWAL application form - you will receive a renewal application by mail prior to expiration of your license
  • Pay appropriate fees
  • Submit a Continuing Education Affidavit (PDF Form)for Reporting Attendance at approved Courses - This form is required bi-annually for all renewal applications.

NOTE: Under the rules a Designer whose license is expired for more than 2 years cannot renew; re-examination is required to become licensed again.
